The University Student Health Insurance plan provides worldwide coverage with the student's choice of provider.

Each semester during the Enrollment/Change period (only) students can:

Add a dependent to Student Insurance coverage

Request a continuation of coverage

Request reinstatement to Student Insurance

Request an exemption from Student Insurance coverage

ATTENTION STUDENTS!

Beginning with the Fall 2012 semester, we require the following forms to be completed online ONLY:

Dependent Enrollment Form

Continuation of Coverage Form

Special Group Enrollment Form (for those who register in Absentia)

Student Health Insurance and UnitedHealth Allies Discount cards
